As a courtesy to our patients, Queensgate Dental participates in many dental insurance plans (PPO). We will submit claims on your behalf and have the reimbursement sent to us. Any conversation with the insurance company is the patient’s responsibility as is any unpaid balance. All service fees are ultimately the patient’s responsibility. For those plans, we are not a participating provider (out of network) we require payment in full by the patient.

For any procedure that may be covered by your medical insurance, we suggest you contact your insurance company prior to treatment. Following treatment, we will provide you with a treatment invoice to submit for reimbursement directly to you.
Please contact your insurance company to confirm that we are an in-network provider. Please bring all insurance information with you to your appointment. This includes the subscriber's name, date of birth, and social security number. Deductibles, Co-payments, and non-covered service fees are expected at the time of treatment. All service fees are ultimately the patient’s responsibility.
